GHI's
disease management programs promote improved health outcomes
and quality of life
An important part of the value that comes with your GHI
coverage is our commitment to helping keep your group
members healthy—and to providing them access to
the care and support they need should they become ill.
In keeping with that commitment, GHI offers disease management
programs that provide participants the support and tools
they need to actively manage their chronic condition and
to improve their health status and outcomes.
GHI
offers programs that address:
- Common
chronic diseases, including diabetes, heart
failure, coronary artery disease (CAD), asthma, and
chronic obstructive pulmonary disease (COPD).
- Less
common, complex chronic diseases, including
Amyotrophic Lateral Sclerosis (ALS), Chronic Inflammatory
Demyelinating Polyradiculoneuropathy (CIDP), Crohn’s
Disease, cystic fibrosis, dermatomyositis, Gaucher Disease,
hemophilia, multiple sclerosis, myasthenia gravis, Parkinson’s
Disease, polymyositis, rheumatoid arthritis, scleroderma,
seizure disorders, sickle cell disease, and Systemic
Lupus Erythematosus (SLE).
Eligible
GHI members are identified as candidates for these programs
through analysis of claims data. Participants and their
providers interact with Nurse Health Coaches and receive
the level of guidance appropriate for the severity of
their condition. Registered Nurses and other licensed
clinicians act as personal health advisors who proactively
assess the member’s health status based on the physician’s
treatment plan and nationally recognized health guidelines,
and provide education about their disease and the best
ways for them to manage it. Member participation is voluntary
and strictly confidential.
Program
goals include slowing the rate of disease progression,
reducing emergency room visits and hospital admissions
when appropriate, monitoring medical and pharmaceutical
service use, reducing employee absenteeism, increasing
productivity, and, most important, improving personal
health and quality of life. Please note that these are
value-added programs that are not underwritten by GHI.
